mso-hansi-font-family: 'Times New Roman';">5.</span><span style="FONT-SIZE: 14pt; COLOR: black; FONT-FAMILY: 宋体; mso-font-kerning: 0pt; mso-hansi-font-family: 'Times New Roman';">习惯性违章、违规。当<span lang="EN-US">35kV</span>系统故障波及到<span lang="EN-US">#4.#5</span>厂变,电压降低造成<span lang="EN-US">#7</span>炉磨煤机、燃油泵掉闸,<span lang="EN-US">#7</span>炉灭火时,运行值班员严重违反运行操作规程,有章不循。在锅炉灭火后,急于恢复,未按规定切断燃料,并进行炉膛通风吹扫,即起动磨煤机致使煤粉及乏气进入炉膛,是造成爆燃的直接原因。《防止电力生产重大事故的二十五项重点要求》及《电业安全工作规程》中明确规定“当锅炉灭火后,要立即停止燃料(含煤、油、燃气、制粉冷气风)供给,严禁用爆燃法恢复燃烧。重新点火前必须对锅炉进行充分通风吹扫,以排除炉膛和烟道内的可燃物质”。但此次事故中司炉未按锅炉熄火事故处理程序操作,而是错误的开启磨煤机运行。<span lang="EN-US"><p></p></span></span></p><p class="MsoNormal" style="MARGIN: 0cm 0cm 0pt;
